To apply for credit approval, please download (PDF) and print the Credit Application Form. Once you have filled in the form, fax it to the attention of the credit department at Simcoe Block (1979) Limited at the number indicated on the application form. We will respond shortly.

Notes on filling in application form.

1.    All information must be printed and legible. (Please use blue or black pen to ensure a clear transmission over facsimile.)

2.    If the account is a personal account then date of birth must be provided.

3.    A corporate application must include the registered corporate name and officers’ names and positions.

4.    Please make sure to include the purpose of your account ie. remodeling, building a house, etc and the estimated monthly purchases that you will be making.

5.    All applications must be signed, witnessed and dated, including having the names printed in the appropriate areas.

6.    A corporate application must be signed by an officer of the company.
